How to Order AssureMDx

1. Set Up Your Physician Account

Complete our New Account Setup Form to register your practice and begin ordering AssureMDx.​​​

2. AssureMDx Test Requisition Form

Download and complete the AssureMDx Test Requisition Form, which includes required fields for patient demographics, relevant clinical history, and ordering provider information.

Please note:  Only a licensed medical doctor with a valid National Provider Identifier (NPI) may order the test.​​​

3. Sample Patient Reports

To better understand the clinical utility and reporting format of AssureMDx, you may download a combined sample report that includes examples of both positive and negative test results.

4. Sample Collection Instructions 

Upon receipt of the completed New Account Setup Form, sample collection & transportation kits will be delivered to the physician office. Urine samples should be collected using collection container provided within the kit to ensure sample integrity during shipping. Instructions are included in the kit and can also be downloaded here:

5. Shipping the Sample for Testing 

Ship the specimen to our laboratory, using the prepaid mailer included in the kit, on the same day as collection.
